Emergency Cards are kept on each child to assist us to contact you in case of an emergency. It is very important that you keep us informed with "up-to-date" information so we may reach you in case there is an emergency. Please fill out both sides of the emergency card and send it back to your childs teacher promptly. State Mandated Physicals Pennsylvania School Law requires students to have a physical exam in Kindergarten, 6th and 11th grades. Please have these physicals done by your family physician and returned to school as soon as possible but no later than October 1, 2007. If you do not have a family physician, Dr. William Ware, our school physician, can perform the physical here at school. I will send out letters informing you of when the doctor will be available. You may be present at the physical if you wish. State Mandated Dentals- Pennsylvania School Law requires students to have a dental exam in Kindergarten, 3rd and 7th grades. Please have these dentals done by your family dentist and returned to school as soon as possible but not later than October 1, 2007. If you do not have a family dentist, Dr. Rosenthal, our school dentist, will perform the dentals here at school. I will send out letters informing you of when the dentist will be available. You may be present at the dental if you wish.

Medications If your child needs to take any medication during school hours you must have a note from your family physician. The note must contain explicit directions as to medication, dosage, exact times of administration, and possible side effects. Over the counter medication such as cough medicine also need to have a doctors note. If your child is on antibiotics three times a day you can give them a dose before they come to school, when they get home, and before they go to bed.
